Community Health Improvement Plan (CHIP)

Work in Progress

Background

The Andrews County Health Department (ACHD) is in the process of developing a Community Health Improvement Plan (CHIP) for Andrews County for the next 3-5 years. The CHIP is a long-term, comprehensive effort to identify and address public health problems in a community. It will be based on the findings of community health assessment activities and is one step in a process to improve the overall health and quality of life for the community. The community health improvement plan will be developed collaboratively and define a vision for a Healthy Andrews County.

Community Health Assessment

ACHD is currently conducting a Community Health Assessment (CHA), which is a large component of the Community Health Improvment Plan (CHIP). A CHA is important in the process of improving overall community health because it provides comprehensive information on the community’s current health status, needs, and issues. Information gathered and partnerships developed through the process will guide community stakeholders in the development of a Community Health Improvement Plan (CHIP). The CHA will consist of the following activities:

1. Creating a Community Health Profile - Completed

2. Community Health Survey - Completed

3. Community Listening Sessions - Upcoming June 23rd & 24th (Sign Up Here!)
